Проводится кампания прикрепления населения к поликлиникам.
Приказ Министерства здравоохранения Российской Федерации от 21 декабря 2012 г. N 1342н г. Москва "Об утверждении Порядка выбора гражданином медицинской организации (за исключением случаев оказания скорой медицинской помощи) за пределами территории субъекта Российской Федерации. в котором проживает гражданин, при оказании ему медицинской помощи в рамках программы государственных гарантий бесплатного оказания медицинской помощи"
С этой проблемой сталкивается огромное количество трудовых мигрантов: им очень сложно получить бесплатную медпомощь в городе, где они живут и работают, но не имеют постоянной регистрации. Новый приказ Минздрава утвердил Порядок выбора гражданином медицинской организации за пределами региона его проживания, который снимает все вопросы.
Этот документ дает людям четкие инструкции, как действовать, чтобы "прикрепиться" к выбранной поликлинике на новом месте. Прежде всего нужно обратиться в выбранную организацию с письменным заявлением. Потребуется предъявить паспорт, страховое свидетельство ОМС, указать СНИЛС, сообщить адрес фактического проживания и место постоянной регистрации. Могут понадобится и некоторые другие документы (например, беженцам нужно будет предъявить удостоверение, подтверждающее этот статус).
Кроме того, нужно будет также предоставить данные о медучреждении, в котором человек лечился до отъезда. Решения придется подождать. Два дня, по приказу Минздрава, отводится на то, чтобы новая поликлиника отправила электронный запрос о пациенте в старую, еще два - на то, чтобы дождаться ответа. После этого человека информируют о том, что берут его на обслуживание. Приказ предусматривает, что нового пациента должны познакомить с перечнем всех работающих в медучреждении врачей и дать возможность выбрать, у кого лечиться. Поликлиника самостоятельно запросит копию медицинской карты человека. Отказать ему в приеме из-за того, что у него нет на руках истории болезни, не вправе. В свою очередь, старая поликлиника должна сообщить данные об "открепленном" пациенте в страховую компанию и ФОМС. Это нужно, чтобы избежать "двойного" финансирования разных медучреждений.
Обязательное условие
Главное правило, которое касается каждого желающего бесплатно пользоваться услугами отечественной системы здравоохранения,- наличие полиса ОМС. Обязательное условие беспроблемного получения полиса – гражданство Российской Федерации.
Так что же про прикрепление?
Если вы зарегистрированы и проживаете в одном и том же месте и во время оформления полиса ОМС не высказываете пожеланий к месту медицинского обслуживания, от вас не потребуется никаких дополнительных действий. То же произойдет, если в момент, когда работник страховой компании вбивает ваши данные в полис, вы укажете адрес проживания, отличный от адреса прописки: по умолчанию вы будете прикреплены к поликлинике по адресу фактического проживания.
Если вы выбираете поликлинику (женскую консультацию, стоматологическую поликлинику и т.п.), которая не обслуживает вас ни по месту регистрации, ни по месту проживания, вам необходимо получить согласие этой организации на оказание вам всего перечня услуг. Согласно статье 21 указанного Федерального закона, поликлиника может отказать вам в прикреплении, если ее плановая мощность превышена. В таком случае вам будет предлбжено выбрать иную организацию.
Чтобы получить прикрепление по собственному желанию, вам необходимо обратиться к администрации выбранной поликлиники с заявлением. Подписывая согласие на ваше обслуживание, главный врач организации обязан уведомить об этом вашу страховую компанию.
Если вы прикрепились к поликлинике, которая не обслуживает район, в котором вы проживаете, то вызов на дом в это поликлинике у вас не примут - нужно будет обратиться в ближайшую к месту вашего жительства поликлинику.
Список документов для прикрепления ‚ поликлинике по собственному выбору
- Заявление на имя главного врача организации (бланк должны выдать в приемной, или вы можете написать его в свободной форме, сославшись на Федеральный закон, упомянутый выше);
- страховой полис ОМС + копия;
- копия паспорта.
- Для прикрепления ребенка вам, помимо заявления и полиса, потребуется копия свидетельства о рождении и копия паспорта родителя, подающего заявление.
Если вам отказывают в прикреплении к поликлинике без объяснения причин, во-первых, требуйте письменный отказ, во-вторых, обратитесь в свою страховую компанию: ее сотрудники заинтересованы в том, чтобы помочь вам прикрепиться к медицинской организации.
